All team check-in documents are due by Friday, May 17th

After being accepted into the tournament, you will be required to submit the required paperwork through your GotSoccer account.

Team Check-In is an easy process! Just login to your team’s GotSoccer account, navigate to the Portage Soccer Classic 2019 tournament page within your team account, and click on the “DOCUMENTS” tab. There you will be able to upload your documents (detailed instructions)


  1. Official State/Association Roster - upload PDF document

  2. Tournament Roster (with Guest Players listed) - upload PDF document

    Log in to your GotSoccer team account, from your team's official roster page, choose Portage Soccer Classic from the 'View by Event' dropdown and add guest players using the green "Add Guest Player" button (guests will appear at bottom of the page)

  3. Player/Coach/Manager Pass Cards - upload as a single PDF document

  4. Risk Management Certification for coach(es) and Manager(s) - upload as a single PDF document

  5. Concussion Awareness Certification for coach(es) and Manager(s) - upload as a single PDF document

  6. Concussion Acknowledgement Form - upload PDF document

  7. Medical Release Form for each player (notary not required) - upload as a single PDF document

  8. Waiver of Liability - upload PDF document

  9. Permission to Travel (out of state teams only) - upload PDF document

  10. Guest Player Approval Form for each guest player (if applicable) - upload as a single PDF document

After all required documents have been uploaded, click on the “Notify” button (located in the “Notify Registrar” section under the “Documents” tab) to notify the PSC Registrar that your team’s documents have been submitted for verification. After review, the contact person listed during registration will receive an email indicating acceptance of documents or listing missing information.

Player/coach/manager pass cards MUST also be presented for viewing at each game. NO EXCEPTIONS
