ONLINE TEAM CHECK-IN

All team check-in documents are due by Friday, August 20th.

After being accepted into the tournament, you will be required to submit the required paperwork through your GotSoccer account.

Team Check-In is an easy process! Just login to your team’s GotSoccer account, navigate to the Portage Fall Classic 2021 tournament page within your team account, and click on the “DOCUMENTS” tab. There you will be able to upload your documents.


REQUIRED DOCUMENTS:

  1. Official State/Association Roster - upload PDF document

  2. Tournament Roster (with Guest Players listed) - upload PDF document

    Log in to your GotSoccer team account, from your team's official roster page, choose Portage Fall Classic from the 'View by Event' dropdown and add guest players using the green "Add Guest Player" button (guests will appear at bottom of the page)

  3. Risk Management Certification for team officials on the sideline - upload as a single PDF document

  4. Concussion Awareness Certification for team officials on the sideline - upload as a single PDF document

  5. Concussion Acknowledgement Form - upload PDF document

  6. Waiver of Liability - upload PDF document

  7. Permission to Travel (out of state teams only) - upload PDF document

  8. Guest Player Approval Form for each guest player (if applicable) - upload as a single PDF document

The PSC Registrar will review your team’s documents and the contact person listed on your application will receive an email indicating acceptance of documents or listing missing information.


THE FOLLOWING DOCUMENTS MUST BE AVAILABLE AT EACH GAME

  1. Player/coach/manager pass cards MUST also be presented for verification with tournament roster at each game. NO EXCEPTIONS

  2. Medical Release Form for each player (notary not required)
